Permission Procedure for Videotaped/Recorded Events

In order to avoid potential liability issues with videotapes of speakers and events that the UWP posts to our website or to iTunes, the Publicity Committee has put together the following procedure:

(1) If you are hosting an event and plan to have the event videotaped, please use the attached form to get permission before the event from all participants who will be taped. Earlier versions of the form do not explicitly give us permission to host the videos on websites.

(2) If Mediaworks videotapes the event for you, please make sure that you keep a copy of the signed form, as they’ve been having a difficult time locating forms from past events that they were supposed to keep on file.

(3) Please give the form to Cindy Dufern, who will keep a permanent file of the forms.
